To apply under the Dental Practice category, you’ll need to provide ID that matches the name on your Blue Light Card account.

✅ Accepted proof of eligibility

You can upload any one of the following:

• Payslip showing:

  • Your full name

  • Dental practice or NHS employer

  • Dated within the last 3 months

• Work ID card showing:

  • Your full name

  • Dental practice employer or NHS smart card

• NHS Payment Schedule:

  • Must show:

    • The dental practice you work at

    • Your name as registered on your Blue Light Card account

    • A date within the last 3 months

  • If your name isn’t listed, please also upload a valid payslip

• General Dental Council (GDC) registration number
  👉 Enter this in the ‘Job title’ field during sign-up

 We only need to see documentation that proves your current employment to check that you are eligible.  When you upload your documentation, we recommend that you redact all information other than your name, employer and date.

🦷 Retired Dental Practice?
You're eligible for a Blue Light Card! Here's how to show it:

✅ The easiest option is to upload an NHS Pension P60 or statement, or an SPPA pension letter

Don’t have one of those? No problem – you can also upload:

✅ A pension document from any provider
✅ Proof that you worked in a dental practice for at least 4 years

 We only need to see documentation that proves your retired status/former employment to check that you are eligible.  When you upload your documentation, we recommend that you redact all information other than your name, pension provider, and if you need to provide supporting evidence of former employment you redact all information other than your name, employer and date.
